Skilled-nursing care, family involvement, psychological care and discharge planning complement a well-rounded, individualized program and encourages each patient's transition to their highest functional level. What You'll Do
  • Track and record patient case information, progress, and evaluate recovery.
  • Meet with patient and their families to understand their needs and recommend appropriate care with treatment plans, while providing support and reassurance.
  • Work with team members to ensure discharge planning goals and objectives are developed.
  • Schedule appointments, and coordinate care

What You'll Need:
  • Excellent verbal and written communication with professional phone etiquette.
  • Minimum of 1 year Case Management experience preferred
  • Customer service oriented
  • High attention to detail
  • High School diploma or GED
  • Must be able to pass a pre-employment background check and drug screen
