Health Plus Management (HPM) is a Physician Support Organization and market leader in clinical practice management in the physical medicine and rehabilitation market with 50+ locations serving Long Island, NYC, Westchester, Upstate NY, NJ and Connecticut. HPM provides independent physician practices specializing in musculoskeletal care with professional business development, marketing and back-office support typically seen in much larger organizations, thereby enabling clients to start and/or grow their private practice beyond what might otherwise be feasible (while maximizing clinical service and patient care).

Health Plus Management is seeking an energetic, patient-centric and detail-oriented Chart Prep to support appointment flow by maintaining the records with medical, administrative and ethical requirements. Chart Prep’s are responsible for collecting necessary forms, documents, and patient medical records to ensure a smooth experience before and at the beginning of their appointments.

Responsibilities

  • Create chart folders (includes adhering of all labels and hole punching) for new patients

  • Responsible for reviewing all charts prior to the doctor's appointment to ensure that all medical records are on file

  • Pull charts daily for patients who are scheduled

  • Review and prepare patient charts to include demographic information, previous x-rays, tests, etc.

  • Make sure that the dates of service are accurately recorded

  • Communicate as needed with physicians and other health care professionals to obtain additional information

  • Make sure patients feel welcome as soon as they walk in the door by delivering quality and compassionate care

  • Answer incoming calls, take messages, and complete outreach to patients in a courteous and professional manner

  • Submit files/documentation to physicians and others as requested for review, quality assurance checks, and other purposes

  • Obtain records from outside sources for preview prior to the office visit

  • Respond to written requests for patient information and calls from other facilities

  • Copy patient records requests and/or complete records requests from outside sources, adhering to timelines for completion

  • Collect all necessary paperwork and authorizations needed for billing

  • Scan information to patient's electronic record

  • Complete additional administrative tasks, as needed

Knowledge and Experience

  • 1+ years of relevant medical experience, preferred

  • Knowledge of EMR is a plus

  • Knowledge of medical terminology/coding is a plus

  • Strong computer knowledge and windows program including Microsoft word/excel

Skills and Abilities

  • Bilingual (English/Spanish) required

  • Excellent verbal and written communication

  • Exceptional organizational and time-managements skills - the ability to multi-task is a must

  • Adaptability and flexibility while working in a fast-paced environment

  • Problem-solving ability and aptitude

  • Outcome-focused, with an ability to work under pressure

  • A solution-oriented mindset

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
