Overview
The Private Duty - Live In Registered Nurse delivers safe, effective, and compassionate nursing care in accordance with the nursing scope of practice and individualized Plan of Care. This role involves direct clinical care, timely documentation, clear communication, and coordination with clients, families, and clinical leadership to ensure high-quality service delivery.
Primary Responsibilities
Care Delivery
  • Provide nursing and personal care in alignment with the Plan of Care, collaborating with the Nurse Supervisor or Manager of Clinical Practice. Responsibilities may include medication administration, transfers and positioning, catheter care, and direct communication with clients, families, and clinical staff.
  • Occasionally perform client assessments and supervisory visits as directed.
Documentation and Communication
  • Report changes in client medical status and emergencies promptly to the Nurse Supervisor or Clinical Manager.
  • Complete required clinical documentation accurately and on time using the electronic medical record system, ensuring compliance with regulatory and organizational standards.
  • Communicate schedule changes such as tardiness, vacation, requested time off, change of status, or reduction in hours to Care Coordinators promptly to support appropriate coverage.
  • Confirm weekly schedules with the Care Coordinator to ensure required staffing levels are maintained.
  • Perform additional duties as assigned.
Qualifications• Current, active RN license in the state of practice, in good standing.
• Proficiency with OASIS documentation.
• Minimum of one year of clinical nursing experience preferred; home health experience desirable.
• Strong communication and documentation skills.
Proven ability to work collaboratively as part of a healthcare team.
• Commitment to patient centered, safe, and compassionate care.
  • Must be willing stay at the patient's residence on a 24/7 basis.

Proficiency in tracheostomy and tube feeding care required.
Work Environment
Patient care is delivered in diverse settings, including private homes, assisted living facilities, and clinical environments.
• Regular activities include standing, walking, bending, reaching, and assisting with patient transfers using proper body mechanics.
• Daily travel to patient locations is required.
• Routine exposure to blood, body fluids, and other potentially infectious materials. Universal precautions and PPE are required.
