Quality Improvement RN โ€“ Lightways Hospice and Serious Illness Care (Joliet, IL)

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness is an independent, nonโ€‘profit healthcare provider licensed in 11 counties in northwest Illinois. We have a stateโ€‘ofโ€‘theโ€‘art facility and the first freeโ€‘standing inpatient Hospice Home in Illinois. We have a 35โ€‘year history for providing compassionate professional care to terminally ill patients and their families. We are state licensed and Medicare/Medicaid certified.

We are currently seeking a fullโ€‘time Quality Improvement RN to join our Quality, Compliance, and Risk Management team. This role will coโ€‘lead QAPI initiatives, serve as the clinical lead for CHAP accreditation readiness, and collaborate on policy development and process improvement.

Responsibilities
  • Coโ€‘lead QAPI initiatives.
  • Clinical lead for CHAP accreditation readiness.
  • Collaborate on policy development and process improvement.
  • Educate clinical staff on documentation standards and infection control protocols.
  • Conduct audits and support regulatory compliance.
Qualifications
  • Illinois Registered Nurse with a minimum of twoโ€‘year experience in home health or nursing home/longโ€‘term care.
  • Previous Hospice experience helpful.

Work schedule: Monday โ€“ Friday, 8:00am to 4:30pm.
