Job Summary:

The Supportive Housing Case Worker provides on-going case management services to individuals and families in recovery from substance use disorder. These services are provided either in the office, participants home, or/and in the community to support recovery and housing stability.

Duties and Responsibilities:

  • Establish positive professional working relationships with individuals assessed and served.
  • Provide and coordinate support to maintain housing stability.
  • Provide case management services, including intakes and assessments.
  • Assess and facilitate access to mainstream benefits (SSI, SSDI, SNAP, TANF).
  • Provide linkages to community resources that increase opportunities for economic independence.
  • Teach necessary skills such as transportation, budgeting, etc, that enable independent functioning as needed.
  • Provide training on activities of daily living and life skills, including but not limited to financial literacy, budgeting, nutrition, and parenting skills to individuals in a group setting.
  • Provide advocacy and teach self-advocacy in accessing needed services.
  • Work with participants to create and achieve smart goals with an individual service plan.
  • Completes all required reports and tracking tools for participants, funding sources, and the agency.
  • Ability and willingness to drive agency vehicle if needed.
  • Respond to after-hours work-related phone calls and emergencies.
  • Perform other tasks as determined by the Manager of Supportive Housing Services or other Department/Agency administrators.
  • Seeks and uses management staff for training and supervision.
  • Seeks and attends community trainings to strengthen skills and knowledge.
  • Attends Agency and Department required meetings and trainings.
  • Sensitivity to, and respect for, the diverse world views and different communication styles of individuals of diverse ethnic, linguistic and/or cultures which nurture and shape an organization that celebrates and promotes diversity in its workforce.
  • Highly effective and well-honed verbal and written communication and presentation skills in English; interpersonal skills necessary to persuade and guide both peers and other Agency staff regarding operational matters.

Minimum Qualifications:

  • Ability to deal with diverse populations.
  • Good typing/computer skills.
  • Lived experience preferred.
  • Must have valid driver’s license and willing to use own vehicle to transport clients
