We are seeking a Community Health Worker to connect persons with disabilities to community resources, including medical care, social services, finance, insurance, and other health related needs. Ideal candidate must reside or familiar with Rhode Island neighborhoods. This position requires outreach to different communities, and direct contacts with Medicaid recipients, as well as some office responsibilities.  
Qualifications:
  • Certified Community Health Worker by RICB
  • Ability to communicate in English, and other languages, preferably Spanish
  • Current CNA or Home Care experience a plus
  • Ability to prioritize and multitask
  • Excellent written and verbal communication skills
  • Strong attention to detail
  • ​Strong organizational skills
  • Proficient in the use of computer
  • Clean background check
