Overview

The Licensed Clinical Social Worker completes patient assessments of mental health needs as well as social needs and gaps in social resources and facilitates finding community resources to help patients and families solve for social concerns that affect physical health. This may include access to proper nutrition, housing concerns, in-home care needs, mental health care needs, and family and psychosocial concerns. The LCSW will conduct mental health counseling services with family members with new or known mental health concerns and may include members with substance abuse disorders.

Duties & Responsibilities

  • The Licensed Clinical Social Worker plays a fundamental role in the field of human services, focusing on improving the well-being of individuals, families, and communities.
  • Serving as a compassionate advocate and resource facilitator, they work directly with clients to identify and address various mental health concerns, social issues, such as poverty, abuse, addiction, and mental health challenges. Assessing SDOH gaps and understanding how they affect our patients is essential to a holistic treatment approach to health and wellbeing. Mental health counseling services to be provided to improve patient outcomes and reduce ER visits and hospitalizations.
  • Conduct assessments to understand the unique needs and circumstances of their clients, providing essential support through counseling, crisis intervention, and connecting them with community resources and services.
  • Collaborating with other healthcare providers, staff members, and social service organizations to develop comprehensive case plans and actions that promote client stability and self-sufficiency.
  • Engage in outreach efforts to coordinate services and resources to meet the needs of vulnerable populations.
  • With a commitment to ethical practice and cultural competence, Social Worker practitioners play a crucial part in fostering social justice and enhancing the quality of life for those we serve

Qualifications

  • Social Work license is required in the State of Georgia. Multi-state licenses are a plus.
  • 3+ years of experience in a healthcare setting preferred.
  • A bachelor’s degree in social work is required.
  • Comfort with technology including the Microsoft suite of products
  • Prior experience using electronic health records is a plus.
  • Social work is incredibly dynamic and requires a solid foundation in social work principles, a keen awareness of community resources, and the ability to navigate complex emotional and social landscapes with empathy and professionalism.

The working environment and physical requirements of the job include:

Work is performed in patient homes as well as remotely via home office. Will be required to travel to homes in the Atlanta, GA. and surrounding metro areas.

In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculators, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
