Familywise Counseling, LLC is currently seeking a dedicated and compassionate Full-Time Licensed Clinical Social Worker (LCSW) to join our remote team serving patients in a hospice care setting. This role will be predominantly remote, allowing the LCSW to work from home while engaging with patients through teletherapy and virtual care sessions. However, our hospice center actively collaborates with healthcare professionals on-site to ensure a comprehensive approach to patient care. You will become part of a supportive and collaborative team that prioritizes mental health integration within end-of-life care.

• Provide psychotherapeutic interventions tailored to patients with mental health conditions, such as anxiety, depression, and post-traumatic stress disorder (PTSD).
• Conduct comprehensive assessments of patients’ mental health status, focusing on their emotional, behavioral, and social functioning to develop informed care plans.
• Create and implement evidence-based treatment plans that address the unique needs of each patient, ensuring cultural competence in all interactions.
• Collaborate effectively with a range of healthcare professionals, including psychiatrists and primary care providers, to coordinate patient care and optimize treatment outcomes.
• Regularly monitor patient progress, adjusting treatment plans as necessary based on patient feedback and clinical observations.
• Educate and support patients and their families on mental health conditions, treatment options, coping strategies, and available community resources.
• Uphold ethical standards by maintaining patient confidentiality and ensuring informed consent is consistently obtained.

The LCSW will primarily serve individuals in hospice care, who often face profound emotional, psychological, and social challenges. This nuanced environment requires sensitivity and advanced clinical skills to support patients and their families during one of the most critical times in their lives. Your role will significantly affect patients' mental health outcomes, providing them with the support and understanding they need.

• A Master’s degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is essential.
• Minimum of 3 years of experience providing psychotherapy and mental health services, specifically with patients who have mental health conditions.
• Strong clinical skills, including the ability to conduct in-depth assessments and to develop customized, evidence-based treatment plans.
• Excellent communication and interpersonal skills to foster trusting relationships with patients and collaborate effectively with other healthcare providers.
• Proficient use of electronic health records (EHR) and other healthcare software systems for effective patient management and record-keeping.
• A deep understanding of mental health challenges and treatments, coupled with a commitment to continuous professional development.

• Experience in hospice or palliative care settings is highly desirable.
• Familiarity with community resources available to support patients and their families in mental health and end-of-life care.
• Background in trauma-informed care practices.
