Job Description:

Department amp; Purpose
Programs: Responsible for fulfilling LADD’s mission to empower adults with developmental disabilities to live, work and connect; engages directly with developmentally disabled adults and their families based on their prescribed individual needs -
Reports To: Program Manager
Stakeholders:
Internal: People Served  Managers  Direct reports
External:  Families HCDDS teams
Employee Classification: Non-Exempt
Summary:
The Program Coordinator is responsible for all essential functions and supervising Direct Support Professionals at assigned locations delivering services to individuals

Essential Functions:

-Responsible for supervising the day-to-day operations of each home as assigned
-Coordinates and monitors staffing and scheduling
-Covers open shifts as needed o Manages House expenses per budget
-Ensures site safety, specific emergency training, and overall compliance
-Trains staff on specific processes, procedures, and practices
-Ensures daily documentation and reporting
-Acts as community and family liaison for site, which may include new resident intake.
-Coordinates and manages individual services including individual service plans
-Safely transports individuals via car or van o Responds to rare “off hours” emergencies when necessary
-Communicates timely with manager regarding pertinent issues and works collaboratively with peers and other departments
-Communicates timely and accurately with families and guardian’s regarding changes
-Models The LADD Way (TLW) and supervises direct reports ensuring their performance is aligned with TLW and the policies and practices set for the department
-Represent LADD in a positive manner as appropriate
-Stays up to date on industry practices and ensures appropriate ongoing training of current staff
-Keeps up-to-date on all relevant training and certifications Additional Responsibilities
-Serves as back-up to program manager when necessary and fills in for staff as needed
-Serves as an advocate for the organization’s mission and programs inthe community
-Participates in interviewing for new employees at assigned location
-Participates in all CPT meetings
