Position Overview
KaiCare AI is seeking an experienced California Licensed Vocational Nurse (LVN) to support Advance Cardiac Care's growing care management programs.
The Care Coordinator will serve as an extension of the cardiology team, providing ongoing patient engagement, education, monitoring, and care coordination services for patients living with chronic cardiovascular and related conditions.
This role is ideal for an LVN who enjoys building relationships with patients, supporting chronic disease management, and helping individuals stay engaged in their healthcare journey.
Key Responsibilities
Care Coordination & Patient Management
- Manage an active panel of patients enrolled in CCM, PCM, RPM, and RTM programs
- Conduct monthly patient care management encounters and wellness check-ins
- Build strong relationships with patients and caregivers through ongoing outreach
- Assess patient needs, barriers to care, medication adherence, and lifestyle goals
- Reinforce provider care plans and treatment recommendations
- Assist patients in navigating healthcare services and coordinating follow-up care
- Escalate clinical concerns to providers according to established protocols
Cardiovascular Disease Management Support
Provide patient education and support for conditions including:
- Hypertension
- Congestive Heart Failure (CHF)
- Coronary Artery Disease (CAD)
- Arrhythmias
- Hyperlipidemia
- Peripheral Vascular Disease
- Diabetes and other cardiovascular risk factors
Support patients in understanding:
- Medication compliance
- Blood pressure monitoring
- Weight management
- Diet and nutrition recommendations
- Physical activity goals
- Symptom recognition and escalation
Remote Patient Monitoring (RPM)
- Review and monitor patient-generated health data including blood pressure readings, weight measurements, pulse readings, and other applicable biometric data
- Identify trends requiring provider review
- Contact patients regarding missing readings or low engagement
- Assist patients with RPM device education and troubleshooting
- Document patient engagement and monitoring activities according to program requirements
Documentation & Compliance
- Complete timely and accurate documentation for all patient interactions
- Maintain compliance with Medicare and payer-specific care management requirements
- Ensure monthly care management activities meet billing and documentation standards
- Protect patient confidentiality and maintain HIPAA compliance
- Utilize EHR and care management platforms effectively
Required Qualifications
- Active, unrestricted California LVN License
- Minimum 2 years of nursing experience
- Experience working with chronic disease management populations
- Strong patient education and communication skills
- Comfortable conducting patient outreach by phone
- Experience documenting in EHR systems
- Strong critical thinking and organizational skills
- Ability to independently manage a patient caseload
- Reliable internet connection and dedicated home office environment
Preferred Qualifications
- Cardiology, Internal Medicine, Family Practice, Population Health, Case Management, or Care Coordination experience
- Experience with CCM, PCM, RPM, RTM, or Transitional Care Management (TCM)
- Experience working with Medicare populations
- Familiarity with eClinicalWorks (eCW)
- Bilingual English/Spanish
- Remote nursing experience
