Physical Therapist (PT) - Full Time/Per Visit - Home Health

* Sign-On Bonus: $15,000 (Full-Time Only)
About Us:
Frontpoint Health is a fast-growing home health agency committed to delivering personalized, high-quality care. We're currently seeking experienced and compassionate Physical Therapists (PTs) to join our team and help patients recover safely and comfortably in their homes.
What You'll Do
  • Provide in-home physical therapy to patients recovering from injury, surgery, or illness
  • Evaluate patients' physical conditions and develop individualized care plans
  • Implement therapeutic exercises and mobility techniques
  • Educate patients and caregivers on rehabilitation best practices
  • Collaborate with nurses, physicians, and care teams
  • Complete timely and accurate documentation in our EMR (Kinnser/WellSky)

Requirements:
  • Active Texas Physical Therapist (PT) license (Required)
  • 1+ year of home health experience (Required)
  • Reliable transportation and valid driver's license (Required)
  • Familiarity with Kinnser/WellSky (Preferred)
  • Excellent time management and communication skills
  • CPR certification (Preferred)
