Job description

Key Responsibilities

  • Perform comprehensive initial evaluations and re-assessments to determine patients mobility, balance, and functional needs.
  • Develop and implement customized treatment plans aligned with patient goals and care plans.
  • Deliver gait training, range-of-motion exercises, strengthening, and fall prevention strategies.
  • Educate patients and caregivers on home safety, adaptive techniques, and proper use of assistive equipment.
  • Collaborate closely with PTAs, RNs, OTs, and interdisciplinary team members to ensure coordinated, high-quality care.
  • Accurately complete documentation in the EMR system per Medicare and agency guidelines.
  • Utilize OASIS for documentation and assessment accuracy; prior experience with OASIS is required.
  • Experience with EMR systems such as Kinnser (WellSky), or similar platforms is preferred.
  • Monitor patient progress, communicate changes in condition to the care team, and adjust care plans as needed.
  • Supervise PTAs and aides in accordance with state and federal regulations.

Qualifications

  • Current California Physical Therapist (PT) license.
  • Minimum of 1 year of experience in a Home Health setting, including use of OASIS documentation.
  • Proficiency with EMR systems (WellSky/Kinnser preferred).
  • Strong interpersonal and communication skills.
  • Ability to work independently while maintaining strong collaboration with interdisciplinary teams.
  • Valid drivers license and reliable transportation.
