You will collaborate with a multidisciplinary team of healthcare professionals, ensuring that patients receive comprehensive treatment tailored to their unique needs. • Conduct thorough bio-psychosocial evaluations of patients to identify barriers to treatment, enhancing their overall health status and resource utilization.
• Collaborate closely with medical staff to develop comprehensive care plans that support the mental and physical health needs of patients, thereby improving treatment adherence.
• Apply individual and group treatment modalities effectively, continuously assessing the course and results of therapy to optimize patient outcomes.
• Maintain detailed and accurate documentation of social work activities in patients' medical records in accordance with departmental standards and HIPAA regulations.
• Provide education and guidance to patients and their families to ensure understanding and adherence to treatment plans, bolstering their journey toward better health.
• Facilitate advocacy efforts for patients and their families, identifying community resources that enhance social support and empowerment for continued self-determination.
• Stay informed about community resources and services, effectively integrating them into patient care where necessary.
• Engage in lifelong learning to enhance professional skills and expertise, participating in ongoing training relevant to clinical responsibilities.

As an LCSW at Jimenez and Associates, you will work with a diverse patient population, including individuals with varying degrees of mental health challenges and socioeconomic factors. Your role will focus on providing culturally competent care that respects and addresses individual differences, particularly as they relate to age, development, and unique social patterns. Building relationships with patients and their families is paramount, as it fosters trust and enhances therapeutic outcomes.

• A Master’s Degree in Social Work from an accredited graduate school is required.
• A current license from the State Board of Social Work Licensure at the Licensed Clinical Social Worker (LCSW) level.
• Proven knowledge and skills necessary to provide effective care while considering the aging process and human developmental stages.
• Strong understanding of cultural patterns that influence patient care.

Ideal candidates will possess between six months to one year of clinical experience within a health or mental health setting. Prior experience working with populations similar to those served within our clinic is highly desirable. Training in addressing the needs of specific patient demographics will significantly enhance your effectiveness in this role.

This role offers a competitive annual salary in the range of USD 85,000 to 100,000, commensurate with experience.
