What you'll do
  • Conduct Comprehensive Health Assessments via telehealth

  • Complete risk adjustment documentation (HCC coding)

  • Close HEDIS care gaps during patient visits

  • Document visits using ICD-10 and CPT II codes

  • Review patient history, medications, and preventive needs

  • Deliver clear care plans and follow-up guidance

Must-haves
  • Active Illinois Nurse Practitioner license

  • 1+ year Family Medicine or Internal Medicine experience
