Hospice of Marion, a part of Empath Health is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve in Marion county.

Schedule: Monday-Friday 8 a.m.-5 p.m.

As part of Empath Health, we help neighbors with advanced illness live as comfortably and meaningfully as possible, while supporting their families and caregivers with respect, guidance, and dignity

What you'll Do

  • Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
  • Provide individual and family counseling, crisis intervention, and bereavement support.
  • Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
  • Assist with financial resources, Medicare/Medicaid, and long-term care planning.
  • Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
  • Support patients and families with advance directives and memorial planning.
  • Educate the community on hospice care.

What You'll Need

  • Education & Experience: Minimum one year of social work experience in a healthcare setting (internships may qualify).
  • Licensure: Must hold one of the following Florida licenses or License eligible (Registered Intern) within 90 days of hire:
  • Social Work (LCSW) or Social Work (RCSWI)
  • Mental Health Counselor (LMHC) or Mental Health Counselor (RMHCI)
  • Marriage and Family Therapist (LMFT) or Marriage and Family Therapist (RMFTI)
  • For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
