We are seeking a compassionate, community‑focused Community Health Worker (CHW) to support patients in accessing care, addressing social determinants of health, and improving overall wellbeing.Position SummaryThe Community Health Worker plays a vital role in connecting patients to resources, providing education, and building trusting relationships that support healthier outcomes.
