YOUR TOP RESPONSIBILITIES

  • Conduct phone screens to determine prospective clients' eligibility for the RAP Program, gathering detailed information to support appropriate placement.ย 
  • Complete intake appointments, including obtaining required documentation, releases, and consent forms while ensuring accurate and timely data entry into the Mission's database.ย 
  • Schedule case management appointments and other follow-up services to support a smooth transition into care.ย 
  • Conduct property searches and urine analyses in accordance with program policies and procedures.ย 
  • Connect clients with appropriate community resources and coordinate external referrals when program eligibility requirements are not met or different levels of care are needed.ย 
  • De-escalate crisis situations and communicate effectively with clients, families, and team members to promote positive outcomes.ย 
  • Build collaborative relationships with community providers and internal teams to ensure continuity of care and service delivery.ย 
  • Demonstrate understanding and comply with all relevant legal, regulatory, and compliance requirements, company policies and procedures, and the Code of Ethical Conduct.ย 
  • Follow the Mission's Statement of Faith and lifestyle and adhere to the Mission's employee policy manual and guidelines set forth by the CEO.ย 
  • Lead or participate in devotions, prayers, or other organizational activities that promote a credible Christian witness and lifestyle.ย 
  • Perform other duties within scope as assigned.

Requirements

WHAT YOU SHOULD HAVE
  • High school diploma or GED required. Bachelor's degree from an accredited university or college in social work, sociology, psychology, counseling, or a related social service field preferred.
  • Minimum of one (1) year of experience in social services, intake, behavioral health, customer service, or a related field required.
  • Previous experience in intake, scheduling, and coordination of care is strongly preferred.
  • Certification in Peer Support is highly desired.
  • Proficient in Microsoft Office software.
  • Program database experience preferred.
