Job Summary

The Hospice Intake Specialist plays a vital role in coordinating the referral and admission process for patients seeking hospice care. This position serves as a key point of contact for patients, families, and referral sources, ensuring a smooth, timely, and compassionate transition into hospice services. The Intake Specialist is responsible for gathering and verifying patient information, coordinating with clinical teams, and maintaining accurate documentation throughout the intake process.

Key Responsibilities

  • Referral Management: Receive and process hospice referrals from hospitals, providers, and families, ensuring all required documentation is complete and accurate.
  • Patient & Family Support: Communicate clearly and compassionately with patients and families regarding hospice services, eligibility, and the admission process.
  • Data Entry & Documentation: Accurately enter and maintain patient information in the EMR system, ensuring all records are current and compliant.
  • Insurance Verification: Verify insurance coverage, benefits, and hospice eligibility; collaborate with billing teams as needed.
  • Care Coordination: Work closely with nurses, social workers, and external providers to facilitate timely admissions.
  • Regulatory Compliance: Ensure all intake processes meet Medicare, Medicaid, and state regulatory requirements.
