In your role as an LPCC, you will:

• Assist primary care providers by identifying, treating, and managing patients' mental health and psychosocial challenges as a key member of the primary care team.
• Conduct comprehensive patient intakes, including detailed diagnostic and functional evaluations, to help inform individual treatment plans.
• Provide focused interventions for patients requiring mental health services, tailored to their unique needs.
• Guide primary care providers on which patients may benefit from continued care within the primary setting versus those requiring specialty mental health services.
• Initiate follow-up communication to monitor patient progress and adjust treatment strategies as necessary.
• Develop relapse prevention plans and assist patients in achieving and maintaining stable mental health.
• Identify at-risk patients and construct proactive strategies to avert deterioration in their condition.
• Coordinate behavioral health care by linking patients with community resources and external providers, with informed consent.
• Document patient progress and treatment details in compliance with agency policies and regulatory frameworks.
• Educate patients on self-management strategies, empowering them to actively participate in their treatment.
• Consult with clinic management and team members regarding behavioral health services and program evaluation.
• Support patients in managing any medical treatment advised by their primary care provider, such as coping mechanisms for medication side effects.
• Ensure adherence to the Code of Ethical Conduct for Clinical Professionals while performing assigned duties.

• Ability to understand the complexities of developmental psychopathology and addiction treatment.
