We're looking for a Per Diem Registered Nurse (Home Infusion) who wants their work to mean something — someone who brings care, accountability, and purpose to what they do every day.

What You'll Do

Responsibilities include:

  • Deliver high-quality home infusion care, including IV insertion, monitoring, and removal, while ensuring patient safety and comfort throughout therapy
  • Monitor patients for adverse reactions, perform assessments, obtain vital signs, and document all care activities accurately
  • Perform venous phlebotomy and manage central line care, ensuring proper technique and infection control standards
  • Educate patients and caregivers on infusion therapy, medication administration, and line care
  • Coordinate patient care and scheduling in partnership with internal teams, agencies, and nursing leadership
  • Communicate effectively with patients, caregivers, physicians, and internal teams to address needs, resolve issues, and support care continuity
  • Review and complete nursing documentation, ensuring accuracy, completeness, and compliance with clinical standards
  • Support care coordination by sharing updates, therapy administration reports, and relevant patient information with appropriate stakeholders
  • Respond to changes in patient condition and escalate concerns as needed to physicians and care teams
  • Maintain professional, ethical, and regulatory compliance in all aspects of care delivery
  • Travel to patient homes and healthcare settings within the assigned area

What We're Looking For

You may be a great fit if you bring:

  • Nursing degree from an accredited program
  • Active RN license (minimum 2 years of experience); multi-state licensure a plus
  • IV certification with experience in central line care and infusion therapy
  • Experience in home care, critical care, emergency nursing, ambulatory infusion, or hospital discharge planning
  • Strong clinical assessment, documentation, and patient care skills
  • Excellent communication and interpersonal skills, with the ability to build trust with patients and caregivers
  • Ability to prioritize, stay organized, and manage multiple responsibilities in a dynamic environment
  • Basic proficiency in Microsoft Office (Word, Excel, etc.)
  • Valid driver's license and willingness to travel to patient homes

Preferred:

  • Bachelor's degree
  • CPR certification
  • Experience with care coordination, staffing, or scheduling
  • Familiarity with medical terminology and home infusion workflows
