Case Manager Job Description includes but not limited to:

  • The Independent Contractor/1099 must possess a valid drivers license and the ability to use own reliable vehicle to transport self and others.
  • The Independent Contractor/1099 is responsible for obtaining and maintaining full coverage Automobile insurance
  • The Independent Contractor/1099 is responsible for obtaining and maintaining Professional Liability (malpractice) Insurance

This position is an Independent Contractor/1099 position, there are no benefits.

The ideal candidate will work in the following counties:

Lake County and the surrounding counties. Clients will be seen in the community, a facility or in their home.

The Independent Contractor/1099 must pass all background checks which include:

National FBI Check; National Sex Offender; Child Protective Services Check; Local Background Checks
