POSITION PURPOSE

The Nurse Practitioner (NP) supports our Adobe Provider Group (APG) is a primary care services team for quality health care for most counties in Southern and Northern Arizona. Centers of excellence include in-home/in-clinic wellness assessments, preventative care, and screening. APG acts as an integrated health solutions company that specializes in customizing programs for insurance groups, providers, hospitals, and families. The APG NP has the overall responsibility of treating assigned members and meeting quality measurements.

As an APG NP, you will be responsible for a variety of tasks which include assessing patient/family psychosocial needs, planning, implementing, procuring services for, and evaluating care needs while meeting with members primarily via telemed. In the APG NP role, your goal is to help improve the quality of life and health literacy by evaluating health status, medical history, current medications, social determinants, and other risks. The APG NP helps to coordinate, monitor, and ensure information is shared in an appropriate and timely primary, across the continuum of care.

While this position provides primarily telephonic/video support, it may require occasional travel within Arizona to provide in-home support to members. Should travel be required, arrangements will be coordinated by management and expenses will be covered.

Typical hours are Monday-Friday, 8a-5p.

DUTIES & RESPONSIBILITIES

  • Work remotely, providing telemedicine visits with HIPAA-compliant video visit platform.
  • Fulfills the obligation of requested and/or accepted assignments.
  • Develop positive relationships with clients, caregivers, providers, and other team members.
  • Interact with peers in a collegial manner, supporting efforts to improve outcomes.
  • Conduct in-home assessments, and primary care on identified patients, following established guidelines.
  • Identify diagnoses to be used in care management and active medical management.
  • Formulate a list of current and past medical conditions using clinical knowledge, judgment, and current findings.
  • Identify quality measures outlined in the contract and ensure completion.
  • Identify gaps in care.
  • Identify internal and external referral needs.
  • Identify urgent and emergent situations for proper intervention.
  • Educate members on topics related to health conditions.
  • Compliant with all HIPAA regulations and maintain the security of protected health information (PHI).
  • Assumes responsibility for personal growth.
  • Develops, maintains, and upgrades professional knowledge and practice skills through attendance at seminars, and conferences and participation in continuing education and in-service classes.
  • Other duties as assigned.

SKILLS & QUALIFICATIONS

  • Three (3) years of nurse practitioner experience is preferred.
  • Experience in the care of the elderly in some geographic regions is preferred.
  • Experience in home health, physicianโ€™s office, or public health is preferred.
  • Bilingual (English/Spanish) is strongly preferred.
  • Demonstrate ability to communicate and work effectively with people of various backgrounds.
  • Possess knowledge of HIPAA, and execution of policies and procedures.
  • Identify and resolve problems in efficient and effective ways.
  • Takes personal responsibility for the quality and timeliness of work and achieves results with little oversight.
  • Adapts to changing business needs, conditions, and work responsibilities.
  • Must be able to exercise good judgment and positively influence and lead others.

Will work with occasional supervision over repetitive tasks. New duties or project-type tasks may receive โ€œstrictโ€ or โ€œdirectโ€ supervision, depending upon the nature of the task(s).

EDUCATION, LICENSES, & CERTIFICATIONS

  • Masterโ€™s degree in nursing is required.
  • Current Licensed Family Nurse Practitioner for Arizona.
  • Must hold an active, current, and unrestricted license. This can be obtained either through direct state licensure or a compact license.
  • Current CPR certification.
  • Current TB Test.
  • Must maintain current CPR certification throughout employment.
