Job description

Advanced Practice Provider (NP or PA)

We are seeking an experienced Advanced Practice Provider (NP or PA) to join our primary care team in a unique, coverage-focused role. This position is designed to maintain continuity of care when primary care providers are out of the office by managing a reduced schedule of acute visits while also supporting clinical inbox responsibilities. The APP will work across our primary care practices to ensure timely access for patients and smooth operations for the care team. In this role, you will provide same-day and short-notice acute care visits across assigned primary care practices. You will also manage provider inboxes during both planned and unplanned absences, including reviewing and responding to patient messages, processing refill requests, and reviewing labs, imaging, and consult notes, as well as coordinating appropriate follow-up care. Success in this role requires strong collaboration with nursing and administrative teams to support continuity of care, as well as clear and consistent communication with covering providers to ensure smooth, effective handoffs. A minimum of two years of clinical experience is required. Gifford Health Care offers competitive compensation and a comprehensive benefits package.
