Job Summary: The RN Case Manager provides comprehensive case management services through patient assessment, care planning, coordination, implementation, monitoring, and evaluation to promote continuity of care and optimal health outcomes.

Required Qualifications:

  • Education: Associate Degree in Nursing
  • Certifications:
    • Basic Life Support (BLS)
    • Certified Case Manager (CCM) or
    • Certified Disability Management Specialist (CDMS) or
    • Certified Rehabilitation Registered Nurse (CRRN) or
    • Certified Occupational Health Nurse (COHN) or
    • Certified Occupational Health Nurse-Specialist (COHN-S) or
    • Advanced Certification in Continuity of Care (ACCC) or
    • Certified Rehabilitation Counselor (CRC) or
    • Nurse Case Manager (RN-NCM) or
    • Care Manager Certified (CMC)
  • License: Current, full, active, and unrestricted license to practice as a Registered Nurse.
  • Experience:
    • Associate Degree in Nursing holders: Minimum of four (4) years of experience in discharge planning or case management.
    • Bachelor of Science in Nursing graduates: Minimum of two (2) years of full-time experience in discharge planning or case management. Full-time employment in a nursing field within the last 36 months is required.

U.S. Citizenship is required

Key Responsibilities:

  • Provide nursing expertise on the case management process, including assessment, planning, implementation, coordination, monitoring, and evaluation.
  • Develop and implement local strategies utilizing inpatient, outpatient, onsite, and telephonic case management.
  • Develop treatment plans that include preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care and optimal wellness.
  • Establish mechanisms for proper implementation of patient treatment plans and follow-up care post-discharge in ambulatory and community healthcare setting.

Work Environment & Schedule: Onsite, Monday - Friday, 7:00am - 4:30pm

Physical requirements: Ability to sit or stand for extended periods while reviewing medical records and working on a computer. Frequent use of standard office equipment and use of clear verbal and written communication. Must have sufficient visual acuity and attention to detail to analyze complex clinical documentation, with occasional ability to move about the workplace and lift up to 1015 pounds.

Compensation & Benefits: $42.92 per hour + H&W
