Job description

) RN MDS Coordinator
The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement.
Responsibilities:
• Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process.
• Coordinates and facilitates care conferences with the interdisciplinary team.
• Coordinates, completes and reviews Minimum Data Sets (MDS)
• Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS process; and completion of applicable sections of MDS.
• Responsible for ensuring all current MDS/PPS updates are implemented and staff is in-serviced.
• Will cross train between PPS and OBRA.
• Conducts regular resident interviews as required by the MDS
• Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed
Qualifications:
• Active RN license in the State of Connecticut, required.
• Knowledge of Quality Assurance/Improvement processes, required.
• Bachelors or Associate degree in a health care related field, preferred.
• Experience in a supervisory position in long-term care, preferred.
• Requires at least 3 months experience completing MDS and completion of MDS training program or equivalent experience and knowledge of PPS
