Full Time

$21.75 - $29.67

Monday - Friday

7:30am - 4:00pm

Work Expectations:

This is an outreach position, meaning more than half of your work will be in the field, at homeless encampments, shelters, and/or in a community house placement. You should be aware that these conditions are often not sanitary.  Being a licensed and insured driver is a requirement of the position and sometimes, employees will be expected to transport clients in their personal vehicles


Working conditions include walking, standing, hiking, bending, cleaning, or walking upstairs for extended periods of times.  It is important for your own safety that you understand you will be expected to maneuver around unsteady terrain. Our Homeless Services are often called to intervene during all weather emergencies including the heat, snow, rain, smoke, and other extreme weather.

Responsibilities:

  • Follow documentation processes and procedures established for Forensic Harps consumers and housing and in accordance with policies and procedures.
  • Provide direct service to consumers of Forensic Harps program.
  • Attend FHARPS admin monthly meeting
  • Attend Bi-weekly Trueblood call
  • Discharge clients (reviewed with Program Manager/consults) in Avatar and PDAMS
  • At the point of enrollment create an electronic Client file in one drive and add documents through the clients course on the program through discharge, maintain the electronic client files.
  • Maintain FHARPS case load tracker excel document in a timely manner
  • Participate with team move in/ out and quarterly inspections
  • Support the turn over of master leasing units to assigned clients as they change phase or discharge
  • Organize and request and maintenance repairs with owners of properties
  • Prepare and attend consults as assigned by phases or as requested by supervisor
  • Assist and support with FHARPS program communications main email and phone line and assigned consumers.
  • Maintain professional phone, email, voicemail etiquette.
  • Work with other care providers of Forensic Harps contract regularly including Forensic Navigators, OCRP, Forensic PATH etc. And other referring agencies.
  • Prepare and submit ETP’s for assigned consumers as requested or notified by supervisor.
  • Create and facilitate approved groups/activities on site at consumer homes and document.
  • Ensure that ethical and professional standards of practice are maintained.
  • Participate in program meetings and activities as required.
  • Demonstrate ongoing commitment to increasing competence and knowledge within the behavioral health field.
  • Provide support and care coordination for Forensic Harps consumers in diverting from the court system using housing first and harm reduction models.
  • Assist in the monthly rent requests, and FHARPS quarterly report.
  • In addition to the above, perform other duties as assigned.


Qualifications:

  • WA State Agency Affiliated Counselor’s Registration.
  • Administrative and clinical experience.
  • Demonstrated ability to work with mentally ill and homeless individuals.
  • Working knowledge of agency and program policies and procedures, contracts, and licensing requirements as required.
  • Availability to work varied hours, including evenings and weekends if needed.
  • Valid Washington State driver’s license; proof of insurance.
  • Satisfactory results from criminal background search.
