About the Role:
Tradition Home Health Care in Brooklyn, NY is looking for a dedicated Homecare Intake Coordinator to manage the intake process for the Michigan Home Help Program. This is an exciting opportunity to play a vital role in connecting clients with the home-based care services they need, making a real difference in people's lives every day.
Responsibilities:
  • Coordinate and process new client intake for the Michigan Home Help Program
  • Collect and verify client eligibility, documentation, and authorization requirements
  • Serve as the primary point of contact for clients, families, and referral sources during the intake process
  • Communicate with Medicaid/state program representatives to facilitate approvals and authorizations
  • Maintain accurate and up-to-date client records in the intake management system
  • Collaborate with care coordinators, schedulers, and clinical staff to ensure seamless onboarding
  • Follow up on pending cases and resolve intake-related issues in a timely manner
Requirements:
  • Prior experience in homecare, home health, or healthcare intake coordination preferred
  • Familiarity with the Michigan Home Help Program or Medicaid home care programs is a strong plus
  • Strong organizational skills with the ability to manage multiple cases simultaneously
  • Excellent verbal and written communication skills
  • Proficiency in electronic health records (EHR) or intake management software
  • Detail-oriented with a commitment to accuracy in documentation and compliance
  • Compassionate, patient-focused attitude with strong customer service skills
