This role is fully onsite, with therapists traveling to patients' homes to deliver care. • Evaluate and assess patients referred by physicians, developing personalized treatment plans that address individual needs and rehabilitation goals.
• Implement evidence-based therapeutic interventions in various settings to assist patients in achieving their recovery objectives.
• Engage in ongoing education with patients and their caregivers to promote understanding of treatment protocols, safety measures, and home exercises.
• Foster an environment of exceptional customer service, ensuring a positive patient experience and addressing any concerns they may have.
• Collaborate actively with a multidisciplinary team to optimize patient outcomes, utilizing the latest research and techniques.
• Create and maintain accurate documentation of patient progress and treatment plans in accordance with legal and professional standards.
• Meet and strive to exceed utilization goals and business objectives set forth by the organization.

The physical therapist in this role will work with a diverse range of patients including, but not limited to, those with orthopedic, pediatric, neurologic, and sports-related conditions. By focusing on home-based therapy, you will have the opportunity to witness the direct impact of your work on patients’ lives, helping them regain independence and improve functional capabilities.

• A Doctorate degree in Physical Therapy (DPT) from an accredited educational institution.
• A valid state license as a Doctor of Physical Therapy (DPT) issued by the Physical Therapy Examining Committee.
• Minimum of two years of clinical experience in a physical therapy setting.
• Current CPR certification.
• Strong interpersonal and communication skills, both verbal and written, enabling effective interaction with patients and healthcare professionals.
• Proficiency in computer skills and word processing, including the use of electronic health records (EHR).
• A commitment to establishing strong relationships with patients to support their journey towards functional improvement.
• A proactive attitude and a desire to engage positively with community members and fellow healthcare providers.

• Experience with home healthcare delivery models.
• Familiarity with a wide array of treatment techniques and modalities relevant to diverse patient populations.

The salary range for this position is between $100,000 and $120,000 annually, commensurate with experience and qualifications. In addition to competitive compensation, we offer a robust benefits package that includes:
