Expedient Home Physical Therapy is seeking dedicated and passionate Physical Therapists (PT) to join our innovative home care team in Avenel, New Jersey. This full-time role involves delivering personalized physical therapy services directly in patients' homes, with an emphasis on creating a supportive and restorative environment for recovery. Work Environment

This role is primarily onsite as you will be visiting patients in their homes throughout Avenel and surrounding areas. Working in a home care setting allows you to foster strong, therapeutic relationships with your patients and tailor your approach to meet their unique needs in a comfortable environment. You will have the flexibility to choose your own hours, typically working Monday through Friday, enabling a healthy work-life balance.

Key Responsibilities
  • Conduct comprehensive evaluations of patients recovering from injuries, diseases, surgeries, or disabilities, identifying areas of need and establishing tailored treatment goals.
  • Design and implement individualized treatment plans utilizing a wide range of therapeutic techniques including therapeutic exercises, manual therapy, and advanced modalities.
  • Regularly monitor patient progress, documenting changes in condition and therapy response, and adjust treatment plans accordingly to optimize outcomes.
  • Educate patients and their families on managing conditions and utilizing prescribed exercises or equipment, ensuring safety and adherence to therapy protocols.
  • Collaborate with a multidisciplinary team of healthcare professionals, including physicians and home health aides, to provide holistic care.
  • Participate in regular professional development through workshops and training to stay current with best practices and innovations in physical therapy.
Patient Population / Client Focus

Your client base will consist of individuals of all ages, including children and seniors with complex health challenges ranging from orthopedic issues to neurological disorders. The focus will be on improving mobility, reducing pain, and enhancing overall quality of life by utilizing evidence-based methods of care that adapt to each patient’s unique situation.

Qualifications
  • Doctorate Degree in Physical Therapy (DPT) from an accredited institution.
  • Must possess a current state license to practice physical therapy in New Jersey.
  • Excellent verbal and written communication skills for effective interaction with patients, families, and the healthcare team.
  • Proficient in computer-based documentation and therapy management software.
  • Strong organizational and time-management skills to manage multiple patient schedules effectively.
