Physical Therapist – Home Health Case Manager

Location: Lombard, IL 60148 Openings: 1 Job Type: Contract | COVID Rate Assignment Dates: February 9 – May 9, 2026

Pay & Schedule

  • Hours: 40 hours per week

  • Shift: 8-hour Day Shift

  • Schedule: Monday–Friday, 8:00 AM – 5:00 PM

  • On-Call: None

  • Weekends: None

  • Holidays: None

About the Role

This is a Home Health Physical Therapist Case Manager position supporting patients in the Central Metro A territory. The PT will manage their own caseload, provide skilled home-based therapy, and coordinate care to help patients safely recover and maintain independence at home. This role offers a steady weekday schedule with no on-call, weekends, or holidays—ideal for therapists who value consistency and autonomy.

Key Responsibilities

  • Perform in-home physical therapy evaluations and treatments

  • Develop and implement individualized plans of care

  • Act as the case manager for assigned patients, coordinating services as needed

  • Educate patients and caregivers on mobility, safety, and home exercise programs

  • Complete timely and accurate documentation per home health standards
