  • Shift: 8-hour Day Shift
  • Schedule: Monday-Friday, 8:00 AM - 5:00 PM

About the Role
This is a Home Health Physical Therapist Case Manager position supporting patients in the Central Metro A territory. The PT will manage their own caseload, provide skilled home-based therapy, and coordinate care to help patients safely recover and maintain independence at home.
This role offers a steady weekday schedule with no on-call, weekends, or holidays-ideal for therapists who value consistency and autonomy.
Key Responsibilities
  • Perform in-home physical therapy evaluations and treatments
  • Develop and implement individualized plans of care
  • Act as the case manager for assigned patients, coordinating services as needed
  • Educate patients and caregivers on mobility, safety, and home exercise programs
  • Complete timely and accurate documentation per home health standards
  • Communicate effectively with physicians, nurses, and interdisciplinary team members
  • Ensure compliance with regulatory and agency requirements
Patient Population
  • Adult and geriatric home health patients
  • Typical diagnoses include post-acute recovery, mobility impairments, and chronic conditions requiring skilled PT
Required Qualifications
  • Active Physical Therapist license in Illinois
  • Home Health PT experience strongly preferred
  • Strong clinical judgment and time-management skills
  • Comfortable working independently in a home-based setting
