The role is primarily onsite, as you will be visiting patients in their homes across the Perth Amboy area. As a Home-Based Physical Therapist, your responsibilities will include:

• Evaluating and treating patients undergoing recovery from injury, disease, or surgery at their residences.
• Developing individualized treatment plans that utilize exercise, stretching, manual therapy techniques, and modern equipment to alleviate pain and enhance mobility.
• Documenting and tracking patients' treatment progress, responses to therapy, and necessary adjustments to treatment goals.
• Training patients, caregivers, and family members on self-care techniques and using assistive devices effectively to facilitate recovery.
• Staying updated on industry best practices by participating in workshops, reading professional journals, and engaging with professional networks.

Our patient population ranges from children to seniors, encompassing various conditions that require specialized care. You will work closely with individuals recovering from surgeries, managing chronic pain, and addressing mobility challenges. Each patient presents unique needs, allowing you to tailor your approach and therapeutic strategies, thereby significantly impacting their rehabilitation and overall quality of life.

To thrive in this position, candidates must meet the following qualifications:

• Bachelor’s Degree in Physical Therapy from an accredited program.
• Current state licensure in Physical Therapy.
• Strong verbal and written communication skills.
• Proficiency in basic computer skills necessary for documentation and communication.
• Demonstrated flexibility and agility in adapting treatment plans as patient needs evolve.

While we value experience in the field, we are open to recent graduates. A Doctorate of Physical Therapy (DPT) is advantageous but not mandatory. Candidates should have at least two years of practical experience in a similar role, along with a current CPR certification.
