Make a difference where it matters most - at home We're looking for a dedicated Physical Therapist (PT) to join our compassionate home health team. Help patients regain mobility, independence, and quality of life - all from the comfort of their own homes.
Main Responsibilities:
  • Conduct patient assessments, create and update care plans, and establish clear, measurable goals.
  • Teach patients and their families exercises and self-care techniques that enhance mobility and promote safety.
  • Complete home safety evaluations and provide recommendations for modifications when necessary.
  • Work closely with physicians, nurses, and the interdisciplinary team to support high-quality patient outcomes.
