Job description

The Nurse Care Coordinator is responsible for providing clinical care management, outreach, and service coordination for individuals living with HIV/AIDS. This role works closely with members, healthcare providers, and community organizations to support continuity of care, identify barriers to services, and ensure access to appropriate healthcare and community resources.

Minimum Requirements:
  • Current RN or LPN licensure in the State of Maine
  • Minimum of three (3) years of progressively responsible nursing and/or healthcare services experience
  • Knowledge of professional nursing practices and current standards of care
  • Strong verbal and written communication skills
  • Ability to work independently while collaborating with healthcare providers, community organizations, and interdisciplinary partners
  • Ability to interpret and apply federal and state regulations, policies, and healthcare program requirements
  • Strong organizational skills with the ability to maintain accurate and timely documentation
  • Ability to develop solutions to complex issues and manage multiple priorities
  • Must meet all applicable federal, state, and local requirements
  • Must be at least 18 years of age
