Here follows information about the position: Pay: $24.00 - $26.00 per hour Job description: One of New York's leading home care agencies servicing all five boroughs and Long Island. Our home care agency employs highly qualified Home Health Aides who are dedicated to provide compassionate, quality care services to our patients. We are currently seeking a "Service Coordinator" for our Queens branch located in Elmhurst

Any home care experience welcome, also we are willing to train if this is new field for you. Responsibilities: Schedules/Assigns caregivers to patient cases. Eager to go above and beyond to accept as many cases as possible in effort to ensure growth and agency profitability.

Interact with families, clients, and caregivers regarding services Follow-up with clients and caregivers regarding coverage, call-offs, and cancellation of staff. Maintain open and current communication with management team of changes in status of clients and caregivers. Actively participate in regular staff meetings to provide guidance on any changes with staffing requirements/needs.

Documents all notes and updates for patients and caregivers into HHA Exchange Scheduling/billing system. Participates in agency on-call when scheduled Reports all incidents to immediate supervisor Ensure that services are provided as prescribed by the client's Plan of Care. Prebilling and working on Call Dashboard daily Performs other duties as needed Qualifications: Minimum High School Diploma Prior experience in home care as a "Service Coordinator" a MUST.

Excellent customer service skills Strong computer and writing skills, including knowledge of Microsoft Outlook Knowledge of HHA Exchange a plus. Bilingual Spanish a plus but not mandatory.
