Key Responsibilities:

  • Provide Compassionate Client Support: Engage with clients in a respectful, positive, and supportive manner during daily interactions.
  • Assist with Daily Living Activities: Support clients with personal care needs—ranging from minimal to full assistance—including bathing, toileting, feeding, dressing, and mobility.
  • Monitor Health Indicators: Accurately obtain and record vital signs, height, and weight, entering data into the appropriate system as assigned.
  • Track Nutritional Intake: Observe and document clients’ food and fluid consumption to ensure nutritional needs are being met.
  • Maintain a Safe & Clean Environment: Report facility maintenance or repair needs promptly to ensure a sanitary, safe, and organized environment.
  • Lead or Support Therapeutic Activities: Facilitate activity groups independently or with assistance, based on qualifications and assignments, and document participation accordingly.
  • Support Client Transitions: Assist with admissions, discharges, and client assessments, including accurate data entry into the electronic health record system.
  • Ensure Safety in Special Care Areas: Supervise clients in designated care areas and report any safety or care concerns to nursing staff immediately.
  • Emergency Response Support: Aid nursing staff in the event of client emergencies, following protocols and ensuring timely communication.
  • Team Collaboration: Work closely with supervisors and the care team to complete shift responsibilities and stay aligned on client needs.
  • Treatment Plan Participation: Contribute to client treatment planning and therapeutic programming under the guidance of licensed professionals.
  • Maintain Training & Competency: Stay current with mandatory trainings to remain compliant and competent in assigned duties.
  • Protect Client Privacy: Uphold confidentiality and ensure proper handling of Protected Health Information (PHI) in accordance with HIPAA and organizational policies.

Education: High School diploma or GED equivalent.


Experience: Basic knowledge of human behavior or general mental health concepts, and independent living skills (e.g. personal grooming, basic nutrition and general health needs.)


Skills & Abilities:

· High School diploma or GED equivalent. Current or recent studies in behavioral healthcare a plus!

· Computer proficiency. MS Office experience required.

· Communicates directly and effectively in a cooperative and collaborative manner.

· Handles sensitive information in a confidential and consistent manner to ensure compliance with HIPAA requirements .

· Facilitate planning meetings with individuals from multiple systems, both internal and external.

· Interact positively with staff and clients and maintain a professional and pleasant demeanor at all times.

· Adapts easily and readily to a changing environment and work priorities.

· Experience / knowledge working in an Electronic Medical Record.
