St. Mary's Medical Center is seeking a Full TimeHeart Failure Coordinator who will serve as the primary clinical nursing resource and central point-of-contact for the Heart Failure Program. Provides coordination and support for nursing workflows, while promoting exceptional patient care, efficient clinic operations, and adherence to organizational policies, evidence-based heart failure management practices, and management directives. Functions as a key liaison among physicians, advanced practice providers, nursing staff, administrative personnel, patients, and external stakeholders to facilitate effective communication, care coordination and continuity of services. Maintains a collaborative working relationship with and demonstrates effective followership to the Physician Operations Manager, supporting departmental goals, operational consistency, and the delivery of high-quality cardiovascular care across all Heart Failure Program clinic locations.
Education Requirements:
Graduate of an approved School of Nursing and hold a current licensure in the State of West Virginia. (RN)
Experience:
- Minimum three years of clinical nursing experience in heart failure clinic support. Experience in cardiology, heart failure management, chronic disease management, or ambulatory care preferred.
Certifications/Skills:
- Demonstrates leadership, communication, organizational, and interpersonal skills.
- Advanced knowledge of heart failure disease processes and treatment guidelines.
- Strong patient education and care coordination skills.
- Excellent communication and relationship-building skills.
- Strong organizational, problem-solving, and workflow management abilities.
- Ability to analyze operational issues and implement effective solutions.
- BLS required.
