Overview

The Nurse Manager Health Care Management role provides oversight and coordination of health care services for the Care Design NY (CDNY) Care Coordination Organization/Health Home (CCO/HH) members.

The Nurse manager is responsible for guiding and supporting the day-to-day implementation, evaluation, and continuous improvement of the Health Care Management Plans. This position is responsible for directly supervising the Health Care Team, each of whom has critical roles in supporting the members and families we support with access to, and ongoing supports related to all health care supports and services. The Nurse Manager plays a key role in the collaboration of the teams' approach to all health care supports and services and works collaboratively with the Health Care Health and Quality department to ensure a holistic approach to care.

Compensation is regionally adjusted within the posted salary range.Responsibilities
  • Directly supervises the Health Care team by clearly and appropriately delegating projects, monitoring teams' overall work performance, and identifying areas for improvement. Supports in the education of all employees about relevant health care standards. Functions as a resource for the health care team and care managers who need assistance with complex or problematic health care issues with members. Leads the complex case reviews, Regional Rounds, and other initiatives as needed to problem solve and assist those members most in need of a comprehensive and creative approach. Support highly complex members via clinical assignment as needed. Supports in the training of staff about chronic condition management, medications, medication management, proactive practices, treatment adherence, monitoring and effectiveness of treatment to best support member health and wellness. Intervene and advocate on members' behalf when their plan of care is inadequate to ensure they receive high-quality, whole person care. Ensure adherence to regulatory requirements such as CMS, NYS DOH Health Home, & NYS OPWDD. Serves as a RHIOS and Medisked subject matter expert. Collects and analyzes data from multiple sources to proactively identify highly complex individuals. Assists in developing, maintaining, and sharing reports and recommendations as prescribed by business needs or upon request. Works in close collaboration with the VP of Healthcare Management & Clinical Support, the Director of Nursing Programs and Supports, and or the Director of Specialized Services to develop effective and efficient workflows to support the overall functions of the Healthcare team. All other duties as assigned.
Qualifications
  • A Registered Nurse licensed to practice in New York state required.
  • A BSN is preferred.
  • 2+ years of experience in health services preferred
  • 2+ years of experience collaborating with people with IDD preferred
  • 3+ years of work experience in a managed care plan or health home is preferred.
  • Experience working collaboratively with various departments in fast-paced environment
  • Demonstrated leadership skills to supervise and train multiple staff on various projects with competing timelines.
  • Proficiency in navigating through electronic record software programs.
