Join Our Team as an RN Home Health Nurse with a Sign-On Bonus!

Phoenix Home Care & Hospice is dedicated to delivering exceptional home health care and hospice services. We believe in providing support to our team of nurses and ensuring they have the right tools and environment to succeed while making a meaningful difference in the lives of our patients.

Location: Topeka, KS and surrounding areas.

Job Responsibilities:

  • Conduct skilled nursing visits for patients in their homes.

  • Engage in direct patient interactions to assess health conditions.

  • Educate patients and families about health management, including CHF, COPD, diabetes, etc.

  • Provide wound care and medication management.

  • Document patient conditions and report significant changes to the physician.

  • Collaborate with a team of other healthcare professionals, including Physical Therapists and Occupational Therapists.

Requirements

  • Current RN Nursing License in the state of practice.

  • Previous home health experience preferred.

  • Ability to work effectively in a fast-paced environment.

  • Excellent organizational and time-management skills.

  • Compassionate attitude with a focus on customer service.

  • Valid driver's license and access to reliable transportation.
