Join Our Team!
Atrio Home Care- a partnership between Holland Home, Brio and Rest Haven- is seeking a compassionate Licensed Practical Nurse (LPN) to provide in-home care throughout the Grand Rapids or Holland territory. This role focuses on delivering high-quality, one-on-one care to clients in their homes within approximately a 40-mile radius of downtown Grand Rapids or downtown Holland/Lakeshore.
Key Responsibilities
  • Deliver skilled nursing care under RN supervision, including wound care, dressing changes, catheter care, and medication/injection administration
  • Follow and support individualized Plans of Care in collaboration with physicians and the care team
  • Provide specialized nursing services such as preventive care (skin care, pressure relief) and chronic disease management
  • Educate clients and caregivers on disease processes, self-care, and health promotion to encourage independence
  • Maintain accurate, timely documentation using the Matrix EMR system (iPads provided)

Schedule & Location
Schedule: Full-time or part-time opportunities available
Hours: Day shift (8:00 AM - 5:00 PM)
Weekend/On-Call/Holidays: None
Qualifications
  • Current Michigan LPN license
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • 1-2 years of acute care or skilled nursing experience (home health preferred)
  • Passion for delivering exceptional care to older adults
