Job description

Primary Accountability

The Community Health Representative promotes Family HealthCare Network services through outreach activities, coordination of community events, and participation in relevant grant initiatives and special programs.

Description of Primary Responsibilities

  1. Responsible for carrying out responsibilities in the following functional areas as assigned: community outreach, enrollment assistance, coordination of FHCN community events, grants, and programs. 
    1. Responsible for achieving departmental goals and metrics for functional areas as assigned. 
    2. Communicates feedback to the supervisor regarding workflows and policies. 
    3. Serves as a subject matter expert in functional areas as assigned. 
    4. Performs high-level administrative functions to comply with regulations and reporting requirements to meet department deadlines.
  2. Responsible for executing outreach activities for the organization 
    1. Participates in community events to disseminate information on FHCN services and programs and provide health screenings. 
    2. Participates in relevant community meetings and committees as a network representative. 
    3. Lead community focus groups and surveys for the network.
    4. Executes network-hosted community events. 
    5. Conducts presentations and educational workshops to raise awareness of network services and programs. 
    6. Maintains relationships with strategic partners and participates in mutually beneficial activities.
    7. Act as a liaison between FHCN and patients. 
  3. Supports consumers with enrollment services and referrals. 
    1. Conduct intake and screenings to determine eligibility for Medi-Cal, Medicare, Covered CA, CalFresh, sliding fee, food pantries, and literacy programs.
    2. Educate consumers on health coverage benefits and rights and assist with navigating services.
    3. Provide ongoing case management to ensure healthcare coverage retention and utilization of FHCN services. 
    4. Completes and maintains all obligatory certifications as required. 
    5. Stays abreast of State and local program updates. 
  4. Executes FHCN programs and departmental grant initiatives.  
    1. Implement programs and grant curriculums at sites and in community settings.
    2. Executes day-to-day operations at food pantries and records daily inventory of food items and related materials.
    3. Receives product deliveries and ensures items are refrigerated and stored appropriately. 
    4. Participates in events to meet program and grant objectives. 
    5. Ensures compliance with all program or grant-related training and certifications. 
    6. Maintains a professional working relationship with all program partners and grant funders.
  5. Responsible for adhering to the Attendance and Absenteeism Policy, recognizing that regular attendance is considered an essential function of all FHCN positions.  Absenteeism is not being at work or failing to attend a paid workshop, training, or event unless the absence is protected by law. 
  6. Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
  7. Performs other duties as assigned.

Description of Primary Attributes

Professional & Technical Knowledge:

Effective June 1, 2026, all individuals hired into the role must:

  1. Possesses work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, and technical skills.
  2. Must have proof of completion of high school with a minimum overall GPA of 2.5 or completion of General Educational Development (GED) with a minimum overall score of 162.5, and health care-related knowledge frequently acquired through completion of a trade school, para-professional, or certificate-type program. If an individual has completed a degree at a higher level than required by the role and had a stronger GPA in that program, they may provide proof of GPA from that degree in lieu of the high school diploma.
  3. Have a minimum of two years of experience in community outreach, educational activities, and customer service. 

Technical Skills:                   

  1. Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs, and other elements.
  2. Ability to use Microsoft Excel to review and compile data, including the use of formulas, functions, lookup tables, and other standard spreadsheet elements.
  3. Ability to create basic presentations in Microsoft PowerPoint.

Licenses & Certifications: Valid CA driver's license and car insurance.

Other Requirement: As a condition of employment at Family HealthCare Network, all employees assigned to provide Covered California education and enrollments must obtain certification as a Certified Enrollment Counselor. This will include successfully completing state-mandated training, passing state certification exams, and passing a Federal Background Check. Any staff assigned to support food programs must obtain a food handlers certification upon hire.

  1. Job duties require compiling information in effective written form, including correspondence, reports, articles, or other documentation.
  2. Effectively conveys technical information to non-technical audiences.

While performing the duties of this position, the employee is regularly required to sit and use repetitive hand movements to type and grasp. The employee is frequently required to stand or walk, and the employee must occasionally stoop, reach above and below shoulder height, squat, climb, lift, and/or move up to 30 pounds. The employee must work in the environmental elements, including cold and hot weather. The employee must travel from site to site and to other non-FHCN locations. 

Pay Scale:

Min Hourly Rate: $21.56

Max Hourly Rate: $30.86
