Job description

The Community Health Navigator (Navigator) is an essential member of the Institute for Population Health (IPH) North Omaha community engagement team. They support community members with navigating the healthcare and social services systems. The navigator provides orientation, referrals, and ongoing support to individuals and families in North Omaha experiencing unmet health and social needs. Partnering with health systems, clinics, and other community-serving organizations, the navigator works with community members to build a supportive network for the improvement of their overall health. Additionally, the navigator conducts community outreach, builds relationships with key community stakeholders, and represents the IPH at community-based events. The Community Health Navigator also provides basic screenings and health education, and makes referrals for ongoing healthcare and disease management as appropriate.
