Job description

JOB DEFINITION:

CN Guidance and Counseling Services works in collaboration with Health and Welfare Council of Long Island to meets the demands of the Patient Navigator, Access to Health Care Program. CN Guidance and Counseling primarily serves the Nassau County population.

The Health Navigator will perform a wide variety of administrative and programmatic duties to support the goals of the Patient Navigator Access to Health Care Program, including but not limited to providing enrollment and navigation services, conducting client satisfaction surveys and documenting information in a data driven way. The Health Navigator will work to increase access to care in the community by assisting consumers in navigating the complexities of the health care system, by providing education surrounding strategies for best utilizing health insurance and health care, and by providing advocacy and enrollment services to at risk populations.

EDUCATIONAL REQUIREMENTS:

Bachelor's degree in health profession from a program approved by the New York State Department of Education preferred.

EXPERIENCE REQUIRED:

  • A minimum of two (2) years' experience working to support at risk individuals in a field based or health care environment.
  • Bilingual preferred.
  • Excellent organizational and prioritization skills, including exceptional time management and impeccable attention to detail.
  • Proficiency in Microsoft Word, Excel, Access, and PowerPoint.
  • Demonstrated excellence in written and oral communication skills.
  • Passion surrounding consumer advocacy for at risk populations and increasing access to health care for these populations.
  • Demonstrated ability to work in a culturally competent and culturally aware manner.
  • Ability to work evenings and weekends as needed.
  • You must have the ability and willingness to travel throughout Long Island. A valid and current New York State driver's license is required and must be on file with the CNGCS' Human Resource Department. You must be designated and remain in good standing as a CNGCS authorized driver.

DUTIES AND RESPONSIBILITIES:

  1. Educate members of the community about the types of health insurance plans offered through the NY State of Health Insurance Marketplace.
  2. Responsible for data entry and tracking to support operational needs of the program and to assist in discerning trends.
  3. Assist consumers in understanding and completing enrollment applications.
  4. Advocate for consumers' access to health care by assisting them in choosing a Managed Care Plan and Primary Care Physician.
  5. Conduct educational workshops and presentations as needed.
  6. Attend and table at community events while representing the goals of the project and the mission of the organization.
  7. Complete and maintain updated records of all required documentation, including but not limited to enrollment information, demographic information, client surveys, and call logs in a detail oriented and timely fashion.
  8. Assist in the completion of program reports.
  9. Assist in training additional team members as needed.
  10. Complete all necessary trainings and comply with recertification standards as mandated by NYS and the organization.
  11. Attend regularly scheduled sessions for program information, updates, ongoing training, and technical support.
  12. Maintain expertise in eligibility, enrollment, and program specifications.
  13. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
  14. Complete QI Training during onboarding and participate in QI refresher training as needed.
  15. Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences. A Culture that makes better decisions, drives innovation, and delivers better business results.
