OverviewClinical Liaison is responsible alongside the Intake Referral Manager for managing the transition of care from acute care setting to post-acute for patients who seek post-acute services with HCF. Areas of focus for Clinical Liaison will be managing hospital referrals, enhancing patient and family satisfaction, streamlining communication, and building relationships with hospital staff.

Responsibilities

  • Must follow HCF Policy and Procedures.
  • Ensure the continuity of care between providers for our patients who are transitioning from the acute care setting to post-acute.
  • Conduct clinical assessments, assist with collecting necessary hospital documentation, complete bedside sales
  • Daily travel between hospitals; occasional travel to area skilled nursing facilities
  • Evening and weekend availability.
  • Responsible for establishing, growing and maintaining relationships with hospital-based referral sources.
  • Maintain core knowledge of our clinical services provided, healthcare industry, and local market conditions.
  • Must be able to handle complexity and change in marketing/sales strategy if the market demands a shift in strategy.
  • Provide clinical communication through various patient updates and testimonial tools available for physicians, hospital staff, and other healthcare professionals.
  • Prepare and maintain Clinical Liaison referral target list and sales goals.
  • Review bed availability and census daily
  • Meet or exceed set number of sales calls to achieve census goals.
  • Pre-call planning, outcomes and next steps needed to achieve desired outcome of all sales calls.
  • Communicate effectively with HCF core selling message to identified referral targets.
  • Communicate with Regional Director of Business Development on any changes in market conditions.
  • Responsible for directly communicating with each care community to ensure smooth transition of each patient to our care.
  • Responsible for establishing, growing and maintaining relationships with hospital-based referral sources, including CEU or other applicable targeted events.
  • Consistently engaged in all accountable care organization meetings, hospital-affiliated events, and other market-driven strategies with acute care market partners.
  • Conduct patient assessments upon request.
  • Maintains company equipment.
  • Respond promptly to clinical needs and routine e-mails.

Requirements

  • Licensed or Registered Nurse preferred, in good standing.
  • Proficient computer skills and digital referral platforms to support position requirements including but not limited to, Microsoft Office, TEAMS and EMR systems.
  • Must have previous experience in care coordination and referral management.
  • Excellent interpersonal communication skills, organizational skills and professional judgment.
