Job Summary: Reporting to the Director of Client Care and agency Administrator/HCA Manager, the Client Care Supervisor (RN) plays a key role in overseeing client care services within a licensed Class B, non-medical home care agency. This role involves assessments of new and existing clients, supervision of client care plans, ensuring regulatory compliance, and supporting caregivers to provide safe, high-quality services. The Client Care Supervisor will assist in assessments, client education, and quality assurance to align with the agency’s mission of delivering compassionate, client-centered care. This position serves as a critical role in ensuring compliance with state-mandated standards to protect the health, safety, and welfare of clients.

Duties and Responsibilities:

  1. Client Care Coordination and Oversight:
    • Assist in the development and implementation of individualized care plans in compliance with Colorado regulations and agency policies.
    • Review Physician Attestation forms and care plans at initial enrollment, upon changes in condition, and as requested by clients, authorized representatives, or case managers.
    • Conduct client assessments and ensure care plans remain relevant to changing needs.
    • Serve as a point of contact for clients, families, and caregivers regarding care needs and adjustments.
  1. Regulatory Compliance and Quality Assurance:
    • Ensure compliance with state and federal regulations governing non-medical home care.
    • Perform periodic audits of care plans, service records, and caregiver documentation.
    • Support the agency in meeting Electronic Visit Verification (EVV) requirements for Home and Community-Based Services (HCBS).
  1. Caregiver Training and Supervision:
    • Assist in training programs for caregivers focused on client safety, confidentiality, and professionalism.
    • Validate and document caregiver competency in performing IHSS tasks.
    • Provide guidance and support to caregivers facing client care challenges.
    • Assist in completing the Orientation of new staff to agency policies and procedures as needed.
    • Ensure personal care staff are trained and educated on the agency’s policies regarding Advance Directives and Emergency Procedures.
    • Has received appropriate training for the job duties and passed the skills validation test required of personal care staff.
    • Meet and confer with personal care staff as necessary to advise and counsel on:
      1. Observation and maintenance of the home environment that ensures the safety and security of the consumer.
      2. Assistance with household chores, including cooking, meal preparation, cleaning, and laundry.
      3. Assistance in completing activities such as shopping and appointments outside the home.
      4. Companionship, including social interaction, conversation, emotional reassurance, and encouraging activities that stimulate the mind (for non-Medicaid consumers only).
      5. Assistance with activities of daily living, personal care, and other assignments as included in the service plan.
      6. Completion of appropriate service notes for each visit, documenting services provided, date and time in and out, and confirmation that care was provided according to agency policy.
  1. Client Safety and Risk Management:
    • Identify and report potential safety risks, ensuring a safe environment for clients and caregivers.
    • Support the incident reporting process and assist in investigations and documentation.
    • Consult with clients, authorized representatives, or attendants regarding medical concerns and interventions.
    • Conduct in-home supervisory visits with each consumer at least every three (3) months, or more frequently as needed, to:
      1. Address problem resolution.
      2. Validate caregiver skills competency.
      3. Provide consumer-specific or procedure-specific training for staff.
      4. Observe consumer condition, care, and provider relationship.
      5. Assess consumer satisfaction with services.
    • Counsel staff on difficult cases and potentially dangerous situations.
  1. Leadership and Program Support:
    • Collaborate with the Director of Client Care, agency Administrator/HCA Manager and executive team to enhance client care policies and procedures.
    • Assist in the development of new programs and services to enhance agency offerings.
    • Foster a client-centered culture within the agency.

Qualifications:

  • Active, unencumbered Registered Nurse (RN) license in Colorado.
  • Minimum of 1 year of experience in healthcare, home care, or related settings and experience with Colorado's In-Home Support Services (IHSS) is preferred.
  • Familiarity with Colorado’s Home and Community-Based Services (HCBS) regulations, including Electronic Visit Verification (EVV) is a plus.
  • Strong organizational, communication, and problem-solving skills.
  • Experience with caregiver training and supervision is preferred.
  • Bilingual (English/Spanish) is preferred.


Physical Requirements:

  • Ability to lift and carry up to 25 pounds (e.g., office supplies, files, or small medical equipment).
  • Ability to speak clearly and effectively to staff, caregivers, clients, and regulatory agencies.
  • Ability to travel locally for home visits, staff supervision, client assessments, and compliance checks.
