We are currently seeking qualified candidates for a full time LVN Intake Coordinator position in the Houston area.


Job Description:
 
Primary function is to facilitate the intake referral process, coordinate care with the interdisciplinary team and the referral source.
 


Qualifications:
  • Graduate of an accredited school of vocational nursing
  • Current Texas State license as a Licensed Vocational Nurse
  • Current Texas Drivers License
  • One year experience as a Licensed Vocational Nurse, home health preferred
  • Nursing skills as defined as generally accepted standards of practice
  • Good interpersonal skills
  • Proof of Hepatitis profile
  • Experience with the intake process and procedures
  • Experience in verifying insurance, Medicaid and Medicare
  • Experience with Microsoft Office (Excel, Word) and Microsoft Outlook - required
  • Detail oriented and highly organized
  • Effective written and verbal communication
  • Clean background and drug screen
  • Reliable transportation
