Key Responsibilities

• Conduct thorough evaluations of patients in their home settings, determining individual needs based on their conditions and lifestyle.
• Develop and implement personalized treatment plans leveraging a variety of therapeutic techniques, including exercises, manual therapy, and modalities to alleviate pain and facilitate recovery.
• Regularly track and document patient progress through detailed records, ensuring effective communication with patients and their families about treatment efficacy and adjustments.
• Reassess and revise treatment plans based on patient feedback and clinical outcomes, striving to meet evolving therapeutic goals for each individual.
• Educate patients and caregivers on the proper use of assistive devices and self-management techniques to promote independence and self-care.
• Ensure compliance with all relevant health regulations and maintain a safe, professional environment while providing compassionate care.
• Engage in continuous professional development by attending workshops, contributing to professional networks, and staying updated with latest therapy advancements.

Patient Population / Client Focus

You will primarily work with patients recovering from injuries, surgeries, or chronic conditions such as arthritis, neurological disorders, and musculoskeletal injuries. Qualifications

• Valid Doctorate in Physical Therapy (DPT) from an accredited program.
• A current and valid physical therapy license in the state of Virginia.

• Proficient in using electronic medical record systems and standard office software for documentation and communication.

Preferred Experience

While a minimum of 2 years of clinical experience as a physical therapist is preferred, we welcome applications from new graduates who have a strong desire to learn and grow in a home care setting. Mentorship will be provided to ensure you are confident in your role and equipped to meet our patients' needs.

Compensation and Benefits
