Job Description
We are seeking a dedicated and experienced Registered Nurse to join our team supporting individuals with intel lectual and developmental disabilities. The ideal candidate brings strong clinical skills and hands-on experience with DBHDD standards, HRST assessments, updating HRST, Proxy scoring, IDD Connect, HCBS waiver requirements, LPN oversight, and maintaining current and accurate health care plans.ย

Key Responsibilities
ยท Complete, update, and maintain HRST assessments in alignment with DBHDD requirements
ยท Ensure timely HRST updates when health status, medications, or risk factors change
ยท Conduct Proxy assessments and ensure accurate scoring and documentation for DSPs
ยท Develop, maintain, and update current and accurate health care plans based on assessed needs
ยท Review health care plans regularly to ensure they reflect the individualโ€™s present condition and supports
ยท Navigate and update IDD Connect for service authorizations, documentation, and compliance
ยท Apply knowledge of HCBS waiver standards to ensure service delivery meets regulatory expectations
ยท Provide clinical oversight, guidance, and support to LPNs
ยท Monitor health status, risk indicators, and clinical needs of individuals served
ยท Collaborate with interdisciplinary teams to implement person-centered care plans
ยท Train and support direct support professionals and caregivers on health-related protocols
ยท Participate in audits, quality reviews, and continuous improvement initiatives
ยท Ensure compliance with DBHDD regulations, HCBS requirements, agency policies, and best-practiceย  standards

Qualifications
ยท Active Georgia RN license
ยท Proficiency in updating HRST
ยท Knowledgeable in Proxy assessments, IDD Connect, HCBS requirements, and maintaining accurate health care plans
ยท Prior experience providing oversight to LPNs (preferred)
ยท Strong assessment, documentation, and critical-thinking skills
ยท Ability to work independently and collaboratively
ยท Commitment to person-centered, high-quality care
ยท Ability to pass a background check
ยท Experience with Therapy and IDD Connect a plus
ย 
Experience
ยท 3-5 years of experience working with people with mental disabilities and IDDย 
ยท Minimum of 2 years' experience with DBHDD policies, HRST processesย 

Education
Required Minimum Education:
ยท Associates Nursing or Diploma (Nurse) Nursing; orย 
ยท B.S. in Nursing (preferred).
