About the Role:

We are seeking a dedicated and compassionate LPN/RN to join our 3rd Shift team at The Greens at Cabarrus, a leading skilled nursing facility in Concord, NC. As an LPN/RN, you will play a vital role in providing high-quality patient care while working collaboratively with our interdisciplinary team.

Responsibilities:

• Provide direct nursing care in accordance with individualized resident care plans
• Assess resident conditions and communicate changes to the physician and interdisciplinary team
• Administer medications and treatments as ordered
• Supervise and support Certified Nursing Assistants
• Complete accurate and timely documentation
• Collaborate with physicians, therapists, and other healthcare professionals to ensure quality resident care
• Promote a safe, compassionate, and resident-centered environment
• Ensure compliance with all state, federal, and facility regulations

Qualifications:

• Active, unencumbered North Carolina LPN or RN license
• Current CPR certification
• Long-term care or skilled nursing experience preferred
• Strong clinical assessment, communication, and organizational skills
• Compassionate, dependable, and team-oriented
