Job description


JOB SUMMARY

This position is responsible for assisting consumers in locating and assisting with maintaining housing opportunities for program participants within the TBRA program. This position is also responsible for developing, locating, and maintaining relationships with area landlords and identifying appropriate federal, state, and local housing resources. This position will also maintain all aspects of the HOPWA-TBRA program for program participants. This position will assist clients accessing Ryan White direct service funds based on the need and usage of the client.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • On-going assessment of consumer’s housing needs.
  • Complete TBRA-needed programmatic visits and paperwork to ensure the consumer has assistance with housing needs and barriers.
  • Assist consumers with accessing Ryan White services and other community resources based on client needs.
  • Create Ryan White and Housing Care Plan for the consumer and work with the client on the goals of the plan.
  • Providing Referrals to internal and external resources within the communities as needed.
  • Identify and develop relationships with area landlords.
  • Identify housing resources through direct contact and Internet-based research.
  • Completes reports and documentation as required.
  • Develop partnerships with federal and state-funded housing programs to create innovative housing strategies.
  • Participates in program-related training, Ryan White, community, or HUD training, as needed or required.
  • Complete necessary case notes, paperwork, and data entry for services provided within the required time frames.
  • Performs related duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

This position requires knowledge, skills and abilities equivalent to:

  • Bachelor’s degree level and at least one year of case management experience working with individuals living in a community setting is required.
  • Housing/HUD knowledge is a plus.
  • Have a demonstrated willingness to be responsive to the needs of the target population, excellent verbal and written communication skills
  • Ability to function as part of a team. Have strong decision-making skills. This position needs to be able to function as a team and handle a flexible work environment.

Must have:

  • Current driver’s license issued by state of residence
  • Current Vehicle Inspection decal
  • Current vehicle insurance
  • Current vehicle registration in state of residence
  • Satisfactory Motor Vehicle Report
  • It requires the incumbent to have a valid Louisiana Driver’s license and be insurable under the VOA/GNO, Inc.’s vehicle insurance. Must be able to pass a criminal history investigation, and be able to obtain Cardio Pulmonary Resuscitation, First Aid certifications.

PHYSICAL REQUIREMENTS:

The employee’s work is performed at various locations where services are provided to consumers. Handle the demands of extensive driving throughout the five-parish service area.
