Position Summary and Qualifications

The role involves delivering direct patient care in line with physician directives while adhering to established standards and organizational policies. You will be responsible for implementing care plans, administering treatments, and collaborating with team members to achieve positive patient outcomes.

Key Responsibilities:

  • Adhere to the care plan, administer treatments, and collaborate with team members to ensure positive patient outcomes.
  • Provide direct patient care as outlined in the State Nurse Practice Act, including specialized responsibilities related to end-of-life care.
  • Assist in coordinating hospice services, effectively communicating identified needs to the interdisciplinary team, which includes Hospice Aides, the Director of Clinical Services – Hospice, and Chaplains.
  • Deliver appropriate pain and symptom management.
Qualifications Required:
  • Basic Cardiac Life Support certification must be obtained within 15 days of starting employment.
  • Registered Nurse (RN) license.
  • Bachelor's Degree in a relevant field.
